Telephone Visit

 

A telephone visit occurs over the phone. You can call into the clinic to schedule a one-hour window of time for your doctor to call you to discuss your health concerns and needs. Your doctor will have access to reference your electronic health record during the call.

INSTRUCTIONS
1. To schedule a Telephone-Visit, please call 503-399-2424 between the hours of 7:30am and 5:00pm, Monday-Friday
2. Before your call window is scheduled to begin, please ensure:

  • Your phone is near you and the ringer volume is turned on/up
  • Your phone is charged or plugged in to a power source
  • You are in a quiet space, with good phone reception, that is free from distractions such as a television, pets or other people
  • You have your current medications, or a list of your current medications near you
  • You have necessary aids available (hearing aids, glasses, etc.)
3. Make sure to answer the call from your provider. The call may come in from a blocked number, or number that is not known to you
4. Try not to eat, or chew gum during your call

The following symptoms could be addressed in a Telephone Visit:

  • Back Pain
  • Cough and/or Cold Symptoms
  • Diarrhea
  • Fatigue
  • Headache
  • Heartburn
  • Red Eye/Pinkeye
  • Sinus Problems
  • Urinary Problems
  • Vaginal Discharge/Irritation
  • The following symptoms and needs cannot be properly addressed in a Telephone Visit:

    • A Procedure (please call 503-399-2424 to schedule)
    • A Physical/Annual Check-Up (please call 503-399-2424 to schedule)
    • Emergent Symptoms (please call 503-399-2424 to speak to a doctor, or dial 911 if you are experiencing these symptoms)
      • Chest Pain
      • Shortness of Breath
      • Weakness
      • Confusion
      • High Fever
      • Sudden Bleeding
