Family Medicine

What to expect

  1. Register with a doctor by calling 503.399.2424, completing our online registration form, or visiting any Salem Clinic location.

  2. We'll confirm your insurance is in network, and help you choose a provider.

  3. You'll receive a welcome packet with a couple of quick forms to complete before your first visit.

  4. Your first appointment officially establishes you as a patient with your new provider.

Common questions

Do I need a referral to see a Primary Care physician?

No, you don't need a referral. You can register directly with one of our Family Medicine or Internal Medicine physicians.

What should I bring to my first appointment?

Bring a photo ID and proof of insurance, your completed new patient paperwork, and a list of any medications you're currently taking.

What's the difference between Family Medicine and Internal Medicine?

Family Medicine treats patients of any age, including children. Internal Medicine focuses on adults, often with more complex or chronic conditions.

Can I keep seeing the same physician over time?

Yes — many patients stay with the same Salem Clinic physician for years.

What if my doctor isn't available?

If you message your provider while they're out, a covering provider or their medical assistant will respond within 48 hours. For same-day appointments, if your provider is fully booked or out of office, established patients can call 503.399.2424 to schedule with a covering physician (depending on availability), or be seen at one of our Urgent Care locations.
